Enrollment in the award-winning Chamber Employer Health Plan Qualifies Enrolled Participants for FREE Wellness Testing (by appointment only)!
Here's how to take advantage of this benefit:
- Enrolled businesses and organizations with 10 or fewer enrolled employees and covered adult dependents who wish to participate in the testing, can make an appointment for an upcoming Wellness Testing Opportunity on May 8 (details below). Contact Covenant Health System Corporate Services at (806) 725- 5864. Once appointment(s) have been made, participants will receive specific instructions and guidelines as well as a questionnaire to complete prior to the testing event on May 8.
- If you are a larger enrolled group with 10 or more enrolled employees and covered adult dependents who wish to participate in the testing, Covenant will travel to your place of business to conduct Wellness Testing. Contact Covenant Health System Corporate Services at (806) 725- 5864 for more information.
WHAT: Wellness Testing Event for Groups with Less than 10 Covered Participants
WHEN: May 8, 7 a.m. to 10 a.m
WHERE: Lubbock Memorial Civic Center (specific location confirmed at time appointment is arranged).
HOW: By appointment only; call Covenant Health System Corporate Services at (806)725- 5864 for appointments and more information
WHAT IF: Your group has more than 10 Covered participants? Wellness Testing can come to your place of business! Call Covenant Health System Corporate Services at (806)725- 5864 for more information.
WHAT ELSE: For just an additional $100 per person, persons enrolled in the Chamber Employer Health Plan can arrange to have a potentially life-saving 64-Slice CT Scan, which allows for a 3-dimensional virtual look inside the heart to spot blockages and other potential problems.
